Steps to filling out a soap chart

Subjective
Talk to the patient and ask questions to see what they have been up to. Ask them if any areas are tender or they have pain anywhere. this is the spot where they tell you and you write.
Objective
This is when you write any abnormalities that you see in the patient. Be very specific.
Assessment
This is where you would write how they feel after the massage. If there's any pain or tightness.
Plan
This is where you would write any notes for next time or any preferences that the patient had.
